Joel Cruz DPT, DN
Founder
Joel Cruz is a Teocaltiche Jalisco, Mexico's native who immigrated to Idaho in the early 1990's. He has been a resident of Bingham County since he arrived at Blackfoot in 1991. He worked in agriculture for a few years, then worked construction along with 7 of his brothers and was co-founder of Cruz Brothers Construction in 1997. He became a general contractor and worked as such until 2011. He then started his school journey in the Fall of 2011 at the College of Technology at Idaho State University. He graduated with an associate in applied (AAS) science degree and became a licensed Physical Therapist Assistant (PTA) both in 2016. He then earned a bachelor's in health science (SBHS) in 2017 and a Doctorate in Physical Therapy (DPT) in 2021 also from Idaho state university. He has worked in Acute inpatient, pediatrics, Orthopedics - Outpatient, Home health, long term facilities, and transitional rehabilitation gaining him a wide range of skills and experience treating patients of multiple conditions and pathologies. Since graduation in 2021, he has been working at Rocky Mountain Physical Therapy for Brandon and Amber Hawker with whom he has developed a great friend and employee - employer relationships. He is now providing Physical Therapy services in one of few clinics that provide Physical Therapy in Shelley Idaho. He is also certified in Dry Needling for arthritic pain, cervicogenic headaches, back pain, neck pain, plantar fasciitis, Hip pain, Knee pain and more. He is father of 3 girls and 1 boy who along with his Wife of 29 years are his support system. He loves to play music, sing, and play soccer every chance he gets. Joel is fluent in Spanish and English and is a fist generation college graduate. Joel always smiles, is happy and pleasant to be around. Justin Usher DPT, DN
Physical Therapist
Justin Usher was born and raised in San Leandro, California in the bay area, though his father and wife both grew up in Salmon, Idaho. He has been a resident of Jefferson County since 2017. He worked multiple technical labor jobs from material handling at FedEx, Security for multiple companies and mechanics at his father’s shop. He graduated from Cal State East Bay with his degree in Kinesiology in 2017. He then worked through and graduated with his Doctorate in Physical Therapy (DPT) in 2021 from Idaho State University. He has worked in acute care inpatient rehab at IFCH, with pediatrics, outpatient orthopedic rehab, home health, and long term care facilities, profiting him a broad knowledge base and experience treating a diverse set of patients with a diverse set of conditions and pathologies. He is now providing outpatient Physical Therapy at Shelley Physical Therapy and Fitness Center. He is also certified in Dry Needling, IASTM and cupping, and the Fascial Distortion Model (FDM) framework. He is father of 2 boys and 1 girl, who along with his wife of 9 years are his support system and whom he spends almost all of his time away from work with. He loves to play basketball, softball, lift weights and perform mixed martial arts when he gets the chance. Justin is fluent in Tagalog and English and is the first in his family to receive a post graduate degree. Denise Moreida PTA,
Physical Therapist Assistant
My name is Denise Moreida. I'm a licensed physical therapy assistant. I grew up in Firth all my life. I did sports in highschool, always had a passion for fitness. I graduated from Idaho State university in 2023 with an associate in applied science degree and became a physical therapy assistant. I Have experience in outpatient and acute care. I’m passionate about patients' recovery, teaching them about their own body, condition and treatment plan overall health and wealthness. I am a mother of one and engaged, and have a family dog. Outside of therapy I enjoy traveling, staying healthy, and enjoying family time. I’m happy to be part of Shelley physical therapy and fitness and serve the community of Shelley Idaho.
